Expanding the Evidence: Real-World Insights into CDK4/6i Use in Elderly and BIPOC Patients with Metastatic Breast Cancer

Randomized controlled trials have long served as the foundation for establishing treatment efficacy, but they often fall short in representing the full diversity of patients seen in clinical practice. This is especially true in hormone receptor-positive/human epidermal growth factor receptor 2-negative (HR+/HER2-) advanced/metastatic breast cancer (a/mBC), where elderly populations and individuals who identify as black, indigenous and/or people of color (BIPOC), remain underrepresented in clinical studies.

In a newly updated systematic literature review, Nadia Harbeck, MD; Adam Brufsky, MD; Timothy Pluard, MD; the team at Pfizer (Chloe Grace Rose, Beata Korytowsky, Connie Chen); and the Value & Evidence team at EVERSANA (Endri Jazexhi, Do Hoang Vien Nguyen, Imtiaz A. Samjoo) examine the real-world effectiveness and safety of CDK4/6 inhibitors (CDK4/6i), palbociclib, ribociclib, and abemaciclib, in BIPOC and elderly patients with HR+/HER2- a/mBC.

The findings reinforce the clinical value of CDK4/6i therapies in these populations, demonstrating consistent survival benefits and tolerability. The review highlights the need for more inclusive research, particularly for BIPOC patients and for newer agents like ribociclib and abemaciclib, which currently have limited real-world data in these subgroups.
